How to apply for a medical card

Newborn Babies

In order for your newborn baby to be added to your medical card, you will need to apply for a medical card for them. We can help with this. For this we will need your baby's date of birth and PPS number. Please contact our Practice Manager for further information.

European Health Insurance Card

The European Health Insurance Card or EHIC (formerly the E111 form) allows the holder to access health care services when travelling to other EU or EEA countries. If you wish to apply for the European Health Insurance Card, please complete the form: here
